The form for the payment plan can be downloaded below. The form is a Microsoft Excel spreadsheet, which can be filled out electronically on a computer.  Simply download the form to a computer’s hard drive, fill in the cells with the appropriate information (see below), print the completed form and sign it, mail it to Customer Account Services (the address is listed on the form), and delete the form from the computer so your information is not saved there.

TERMS:

• The calculation of the monthly payment includes all charges (excluding rent, phone and other incidental charges) that may be billed to a student, less any financial aid that may be received.  Financial aid includes any loans, scholarships, waivers or grants that will be applied to the students account balance. 
• NDSU’s rent payments are to be paid on a monthly basis in addition to the Payment Plan Agreement. Other incidental charges are to be paid as they are posted to the student’s account. Students are responsible for monitoring their own balances on Campus Connection.
• The Monthly Payment will be calculated on actual charges.  Additional charges added or financial aid applied after a billing date will adjust future Monthly Payments.
• If financial aid is not received by the regular financial aid disbursement dates, the payment plan will be increased to reflect this discrepancy.
• Statements will be processed on the 20^th of the month with payments due the 1^st of the following month.  Due dates are as follows:

• • Spring 2008 – 1/1/08, 2/1/08, 3/1/08, 4/1/08, 5/1/08
  • Payments are due on above dates, regardless of receipt of statement.
  • Payments 30 days past due will result in a $15 late fee and will result in a hold on transcripts and registration.
  • Payments 60 days past due will result in the immediate cancellation of this payment plan. Upon cancellation, the remaining balance is due immediately and will be subject to a $50 late fee.
  • Failure to pay, or repeated late payments may result in the denial to participate in future payment plans.
• The Payment Plan Enrollment Fee is $25 per semester.
• The student’s signature is required on the form.  If another individual will be making payments, then this signature may be added.  NDSU will need to have a completed FERPA form on file to communicate with the additional signer.
